New Patient Information and Registering with the Practice

An application to join the practice list can only be considered if the list is ‘open’ and if the prospective patient lives within the Practice Catchment Area.

Our catchment area covers the majority of the E17 postcodes and a full list of the streets included in the catchment area is displayed at the reception or you can download them here.

To register please attend at the surgery on either Tuesdays or Thursdays between 2.30pm and 5.30pm or on a Saturday morning between 10.00am and 12 noon.

When attending you must bring with you:

1) proof of residency; bank statement, land line telephone bills (we do not accept mobile phone bills) and these must be dated within the last three months.

2) a medical card (if you have one).

3) Please bring identification papers in the form of a passport/EU Card or Home Office Papers or a Photo Driving Licence.

4) if registering a child please bring the ‘red book’ or full details of the child’s immunisation status.

5) you will need to complete a New Patient Health Check Form for age group 14 years upward (please download here) and a form for new born babies (please download here).

In deciding whether or not to accept a patient, the practice may not discriminate on grounds of disability or medical condition, age, appearance, race, gender, social class, religion or sexual orientation. If an application is refused the applicant will be given the reason for refusal in writing.

Newly registered patients will be offered an appointment for a new patient health check with a Nurse or Health Care Assistant.
